Welcome to the LUNG FORCE Walk Configuration Request form.

PLEASE NOTE: Prior to filling out this survey, please ensure you have the required ROI source code and GL account string for your event. If you don’t have this information, please submit a request to the finance department at sourcecodes@lung.org per the ALA Deposit Procedures. Also note that we are continuing to use the existing source code string structure, but with the new GL string attribute on source codes starting in FY18.

This online form is specifically for LUNG FORCE Walk and Run/Walk event (for events taking place July 2018 through June 2019) configuration requests using the FY19 blueprint. If you are requesting the build of a Fight For Air Climb event, please click here.

Submitting this ticket will result in the creation of a support ticket for the configuration of your event. Please be sure to provide all of the requested information, as any omissions could result in a delayed configuration for your event.
